This Policy Framework sets out the mandatory actions following the death of a person under probation supervision. It describes the information to be gathered, recorded and shared and explains the roles of probation practitioners, their line managers and staff with specific responsibilities for deaths under supervision. It acknowledges the emotional impact that a death may have and describes the process for ensuring that the welfare needs of staff and others are considered. It explains how we use learning from deaths to improve our service and our approach to preventing deaths.
