Use these forms to return annual data from antenatal sickle cell and thalassaemia screening laboratories.

There are 2 forms, with different versions for:

  • high prevalence areas
  • low prevalence areas

Use the area prevalence for NHS trusts guidance to determine which form to use.

This publication was previously published by Public Health England, which closed on 30 September 2021.
