The Word document versions allow you to co-brand with UK Health Security Agency (UKHSA) by adding your logo.
The recording form is for capturing details of your tick encounter to share with UKHSA.
UKHSA’s tick surveillance scheme explains how to take part in the scheme and how to complete the form.
