You need to complete this form to report any actions you took under a licence to disturb wild birds during:

• science, research, education or conservation work
• photographic work

If you’re using the report form to renew your licence, you must include an updated project plan and map.

You need to send this as soon as possible after you’ve carried out the action. Complete a separate form for each species, county and year.
