This memo is for Work and Health Programme (WHP) providers.
Background
Providers will be aware that the WHP programme design has been adapted to take into account a number of short term solutions necessitated by the ongoing Covid-19 pandemic including the introduction of Provider Self-Referrals.
Although self-referrals are a tactical solution that we will bring to a close in October, it’s important that information is accurate as a rejected referral still counts as a submission and one of your allocated monthly volumes.
This memo is about a revised version of the WHPSO1 Self-Referral form and an Aide Memoire to assist completing the form.
Action
The revised version of WHPSO1 has been published. This should be completed manually, as is the current process, using this latest version of the Self-Referral form (WHPSO1 V4). Please delete any previous versions of the form.
We have also issued an Aide Memoire which is a copy of the new form with prompts highlighting what needs completing in each section to avoid rejection. The Aide Memoire is clearly marked for information only and not to be used for self-referrals.
