Funeral directors and crematorium operators can use this service to submit revenue returns and compliance statements

In completing this form, you will be asked to provide information including:
- your name and business details
- data on your business’ revenue
- number of funerals or cremations your business carried out during the reporting period
- compliance statement
Compliance statements must be submitted by a Chief Executive Officer, Managing Director, Non-Executive Director, Senior Director, or a business owner.
By filling out this form, you confirm you are a decision maker at your business and can verify the compliance statement.

When to submit your information
Revenue returns
Revenue returns are due twice a year.
April returns are due by the first working day of April. These cover the period from 1 September to the last day of February.
October returns are due on the first working day of October. These cover the period from 1 March to 31 August.
Compliance statements
Compliance statements are due once a year.
Compliance statements need to be submitted by the last day of April. These cover the period from 1 April to 31 March.
