Checklist for Declaring Document Codes
Once the full set of Document Codes are identified, the Document Codes should be declared using the following steps:
- Any Union codes should be declared first
- Declare the 4-digit Document Code with the following details, as appropriate, for the specific Document Code as not all of these details are required for every code:
- Document identifier (ID),
- Document Status,
- Reason,
- Issuing authority name,
- Date of validity (expiry date),
- Measurement unit/ qualifier, if applicable and
- Quantity
- Use the Appendix 1 completion instructions for the chosen Procedure Code, where given
- Use the Appendix 2 completion instructions for the chosen Additional Procedure Code, where given
- Use the Appendix 5A completion instructions where the code is not covered by Appendix 1 or 2
- Where the Document Code requires specific text to be used, it should be completed as stated in the guidance
