Step 1
If the form is an E401, go to Step 2.
If the form is an E402, go to Step 6.
If the form is an E403, go to Step 11.
If the form is an E411, go to Step 14.
If the form is an E001, go to Step 19.
If the form is a UK-PL E Form, go to Step 25.
Step 2
Note: The EU Member State authority will complete Part B of this form in response to the enquiries. Part A will have been completed by the Child Benefit or Tax Credit Office.

Part B - this will have been completed by the EU Member State authority in response to the Child Benefit or Tax Credit Office enquiries
- Box 6 - this box will be completed for the family members mentioned in Box 4
- part 6.1 - shows the surname, forenames, dates of birth and relationship to the person mentioned in Box 1 of all confirmed children mentioned in Box 4
- part 6.2 - is completed if the family members live at an address other than that stated in parts 2.5 and 3.5
- go to Step 4.

Box 7 - this box will not be completed as it is only used if information has been requested by a Danish, Icelandic or Norwegian institution

- part 4.1 - shows the date the young person has attended the educational establishment
- part 4.2 - shows the dates the school year started and finished
- part 4.3 - shows the type of school and the form of study which the young person is undertaking
- part 4.4 - shows the date when the education is expected to finish
- part 4.5 - shows how many hours are undertaken as study and how many days a week these hours are spread over
- part 4.6 - shows how many hours are undertaken for homework

Box 5 - this box will not be completed as this is only used if information has been requested by an authority in the Czech Republic, France, Latvia, Luxembourg or the Netherlands

Box 6 - this box will not be completed as this is only used if information has been requested by an authority in Germany and the course is less than 10 hours a week

Box 4 - this box shows information relating to the young person attending an apprenticeship
- part 4.1 - shows the date the person mentioned in Box 2 has been undertaking an apprenticeship
- part 4.2 - shows either the number of days or hours a week the person is undertaking apprenticeship and the expected date of completion
- part 4.3 - shows whether the apprentice is receiving an apprenticeship or wage allowance or other benefits. If the apprentice is not receiving an apprenticeship or wage allowance or other benefits, it will show the net or the gross amounts and whether this is paid weekly or monthly
- part 4.4 - shows the name of the place of work
- part 4.5 - shows the name of the person or institution responsible for the apprenticeship
- part 4.6 - shows the address of the person or institution named in part 4.5
- part 4.7 - shows the official stamp of the person or institution named in part 4.5
- part 4.8 - shows the date the form was completed
- part 4.9 - shows the signature of the person who has responded

Step 13
Box 5 - this box will be completed if the responding institution is French
- part 5.1 - shows the name of the body responsible for supervising the apprenticeship
- part 5.2 - shows the address of the person or institution named in part 5.1
- part 5.3 - shows the official stamp of the person or institution named in part 5.1
- part 5.4 - shows the date the form was completed
- part 5.5 - shows the signature of the person who has responded

- is entitled to family benefits and the amount of family benefits paid

- part 6.3 - shows the income of the person or persons mentioned in Boxes 2 and 3

Box 7 - this box shows the family benefit information referred to in Box 6 for each family member including the
- surname
- forenames
- date of birth
- relationship
- place of residence. It may also include a breakdown by family member of the type of benefit, amount and frequency

Box 8 - this box is completed for the person named in Box 2 if family benefits were paid by an employer in the country of residence

- part 8.4 - shows the date the form was completed

Box 9 - this box shows the name, address and reference number of the office responding to the enquiries
- part 9.1 - shows the EU Member State department responding to the enquiries
- part 9.2 - shows the address of the department
- part 9.3 - shows the file or reference number of the customer in the EU Member State
- part 9.4 - shows the official stamp of the EU Member State department which is responding to the enquiries

Box 11 - this box will be completed with information regarding employment and family benefits paid in Poland
- part 11.1 - shows the period mentioned in Box 6 and whether this was paid to either
- the person in Box 3
or - the person in Box 4
and shows if the person- worked in Poland for the period stated
- did not work in Poland for the period stated
- the person in Box 3
- part 11.2 - shows the period of family benefits paid and whether this relates to
- the person in Box 3
- the person in Box 4
and shows if the person- is entitled to family benefits in Poland for the children mentioned in Box 10
- the amount of family benefit paid, if applicable
- is not entitled to family benefits and lists the reasons why
- has not submitted a claim

Box 12 - this box will be completed if further information for any family members was requested
- part 12.1 - shows the name of the family member, the kind of family benefit in payment, and the amount and periods of payment

Box 13 - this box will be completed if the UK authorities offered reimbursement
- part 13.1 - this will be ticked if reimbursement is being requested
- shows the amount of the reimbursement required and the periods for which reimbursement is needed
- shows the name and full address of the Polish authority requesting reimbursement
- ticked if reimbursement is not needed
- part 13.2 - this will be completed if the Polish authority is unable to provide the information requested and will tick the box and provide the reason why when this applies to
