Cancelling your permit or part of your permit is known as ‘surrendering’ or ‘partially surrendering’ your permit.
Fill in this application form if you want to surrender a radioactive substances activity permit for unsealed sources and radioactive waste at a non-nuclear site.
You must also fill in forms:
- part RSR-A, which is about you and your premises
- part RSR-F, to calculate your application charges and make declarations about your application

Form RSR-E4 now applies only to the surrender of radioactive substances activity permits at non-nuclear sites. A new form, RSR-E3, has been created for the surrender of radioactive substances activity permits at nuclear sites. RSR-E4 clarifies how the form should be used when applying for a full surrender or partial. The guidance has been changed to reflect this.
