All applicants must fill in parts A and F. Read the guidance notes and use this part of the form to apply for a new bespoke radioactive substances activity permit.

   Change to guidance to reflect requirements of Transboundary Radioactive Contamination Direction.

   Form and guidance has been amended to remove reference to Article 37 and introduce requirements of the Transboundary Radioactive Contamination Direction.
